Licensed Practical Nurse
Hours: 3pm-11pm or 10pm-9am and weekends.
Full and Part Time Shifts Available
Hourly Rate: $23.00-$25.00
Responsibilities
- Administer medications, provide nursing treatments and gather patient measurements, signs and symptoms that can be used by the RN in making decisions about the nursing care of specific patients.
- Accompany consumers and act as an informational intermediary for medical/surgical or specialist appointments.
- Participate as a team member of the interdisciplinary care team.
- Participate as directed by the RN in identification and prevention of client abuse and identification of or provision of care in emergency conditions.
- Participate in Health Teaching to include, but not limited to, patient teaching regarding signs and symptoms of medication side effects, disease process and management in relation to life style factors.
- Participate in consumer Health Counseling based on the cognitive level of consumer, i.e., the importance of hand washing in infection control.
- Provide nursing treatments relative to restorative care and health maintenance, including, but not limited to: monitoring bowel and bladder training, stormy/wound care. Reporting of signs and symptoms of post-surgical complications and monitoring of chronically ill consumers.
- Participate in Supportive Care, including, but not limited to, chronic pain management, hospice, and palliative care.
- Participate in Medication Management, including, but not limited to, administration of medication, maintenance of MAR’s, timely reordering of medications, the integrity and orderliness of the medication cart and/or room, the appropriate disposition of outdated medications as per policy.
- Maintain documentation as appropriate, including, but not limited to, accurate tickler files, medical records, appointment schedules and nurses notes.
- Performs related duties as requested by RN.
Job Requirements
- Graduate of an accredited Licensed Professional Nursing Program
- Must possess current LPN license to practice in the State of New York.
- At least one year experience working with individuals with intellectual and developmental disabilities is preferred.
- Current CPR certification.
